In every person’s life there comes a time when he/she is in need of an excuse letter from his/her medical practitioner. Hence this note serves the purpose of accomplishing leave from work activities, under some conditions. The condition may be the individual is unable to move due to some injury, or the person is under medical attention for more than two days.

The medical excuse note is written in the name of any individual who indulges in some work activities or even if he/she is a student in college or in university. This note will help him/her to compensate for the absences that occurred due to the accident or unhealthy state of the student. The same purpose will be served for the people indulged in professional life.

Medical excuse notes are simple in nature and are different from one issued note from any doctor to another. It is merely a suggestion from the doctor to whom it may concern that mentioned party is prescribed rest/medicine for the recovery.

The note will generally consist of the doctor’s name, address, and phone number in case of inquiries, date of issuance of the note, patient’s name, excuse from work, from a certain date to mentioned date, reason of absence, medical advice, and signature of the doctor.

This note may be presented by the employee or student at the time of recovery when attending the session, or simply the legal guardian or the parent can take this note to the organization of interest along with the application from the sufferer. This will content will make up for any inconvenience that might have been caused by the employee or the pupil.

Work

Please excuse XYZ from work owing to his severe cervical pain. He has been suffering from the issue on and off for the past few months. The physical stress at work has worsened his condition. He needs to rest for at least a week in order to recover.

I would like to bring to your attention that his pain may shoot up due to certain activities. These include driving and working at the computer. Therefore, driving to the office and working for long hours on a laptop is not recommended. He should, therefore, be exempted from work for a week.

School

My patient, XYZ has been diagnosed with chickenpox. He is currently suffering from high-grade fever and vomiting due to the infection. The school should grant him leave so that he can fight off the infection and regain his health. It is also important for him to stay at home so that he does not spread the disease at school.

XYZ must remain in isolation until he has fully recovered. His next medical examination will take place on (date). His health condition will determine whether he is ready to attend a school or not.

Jury duty

ABC has tested positive for [Disease]. He has been put in isolation with immediate effect. It is strongly recommended that he stays in complete isolation for two weeks in order to prevent the spread of the virus. Regretfully, he will not be able to perform his jury duty on (date).
