The form’s heading can be “Rental Agreement Form.” You can arrange the form in categories or sections that include similar details related to the rental agreement.

Details of parties concerned:

A heading for the parties will include a statement that the form is between the parties noted below. There can be a heading for the landlord under which the name and address of the landlord will be present. A heading for the tenant will include the name of the tenant. You can have an area that notes down the names of any other occupants.

Property:

Under this heading, the address of the property will be included. You can note down the residence type—an apartment or a home. Other details about the property can be included, such as how many bedrooms and bathrooms it has, whether it has a garden, etc.
