I am writing to inform you that, unfortunately, I have been admitted to the hospital due to severe chest congestion. Since I overlooked my cold and cough that I contracted a few days back, the infection has progressed and worsened. As a result, I need to be in the hospital for 2 to 3 days to receive proper treatment. The doctor will monitor my recovery before discharging me.
Considering the situation, I request you for sick leave for a few days. I will rejoin once the doctor gives me the green signal.
I shall be really grateful if someone takes over my tasks during my absence. Kindly ask XYZ to cover for me since he has the expertise to handle my responsibilities. I am extremely regretful for the inconvenience this may cause to everyone at the office. I hope the workflow is not disrupted.
Thank you for your understanding and support.

I am writing to officially request 3 days off from work on account of a cold and cough. It’s a viral infection that may spread to other employees if I continue to attend the office. Further tests will reveal if it’s a coronavirus infection or regular flu. I will inform you as soon as I get the test results.
In the meantime, I shall be available online to join my team for meetings and other work-related tasks. I will do my best to fulfill my job tasks from home.
I have also asked XYZ to perform the site inspections on my behalf since we cannot delay these inspections further. I have guided him and I believe he will do the job well.
Thank you for your cooperation and understanding.
